Focused Postpartum Care (Focused-PPC) is an innovative, integrated group postpartum care, education, and support model delivered to women after birth up to 12 months / one year after delivery. Specifically, women enrolled in Focused-PPC receive postpartum clinical assessments and education for the first 6 weeks, and continue to receive education, measures of vital signs, and peer support for 12 months postpartum.
